These documents can be used to assist your church or organization facilitate 40 Days for Life sign-ups. If interested in participating in 40 Days for Life as an organization, the indicated (*) documents should be completed and sent via e-mail.
*Hourly Form - Use this form to help your members sign up for the prayer vigil by the hour during 40 Days for Life.
*Statement of Peace - Our statement of peace gives details about how we will act when representing 40 Days for Life. All 40 Days for Life participants must sign this document. Please return all signed forms to the coordinator.
*Adopt a Day Packet - Contains the Adopt a Day sign up form, our Statement of Peace, and a sign up sheet for the day.
*Adopt a Day - This form can be used if your church or organization commit to all 12 hours of prayer in one of the 40 days.
